Alameda County Care Connect Problem Solving Learning Community!

The Alameda County Care Connect Problem Solving Learning Community is open to representation from any interested organization providing services to low-income vulnerable populations in Alameda County, regardless of formal relationship the Alameda County Care Connect. The most appropriate participants will likely be frontline workers, managers, quality improvement staff, or operations employees who encounter system issues and want a place to work across sectors to find lasting solutions.

This convening will bring together a wide range of provider organizations to identify systemic barriers experienced in care coordination and care transitions in Alameda County, and collaboratively create and test solutions to those challenges. Areas of focus may include referrals, linkage to services, communication between organizations, issues of capacity, etc. The Learning Community will include presentations by provider organizations to clarify how to access their services, orientation to new sources of services such as the Housing Resource Centers, and discussions to gather feedback on system improvement work occurring through other elements of the Care Connect program. All of these threads will be the building blocks of the system of care coordination that Care Connect is clarifying, improving the efficiency and effectiveness of the care we provide to our clients as a community of practice. Refreshments will be provided.
